UIL Athletics Forms

There are 5 necessary athletic forms required by Shiner ISD and the UIL. You can find these 5 forms below, and many other specialized forms are available at the UIL website.  

 

Some forms on this page are fillable forms and require a compatible web browser (Chrome, Firefox, Edge, Safari on laptop or desktop computer) or the free Acrobat Reader software (click here to download). Fillable forms allow you to type directly onto the form. You can fill out all sections of the form on your computer except for any field requiring a signature.  You can then print the form and sign it.  If a printer is unavailable, you can download the form to your computer and email it or save it to a USB drive for completing and printing elsewhere.  

 

Many of the forms (except the Preparticipation Physical Evaluation & Medical History form) are available in Spanish, but be aware that Spanish versions are not fillable, and will require printing and completing by hand.
